The vblank code was broken in the previous version of the driver, and it might have helped with this game. If that is the case, then it's still Quake 1's fault, and I still recommend that you try adjusting the settings. In particular, if there is a vsync option, set it to off.
Additionally, what other changes have you made to your machine since the last time that you played? For example, did you disable DMA in Picasso96 as
discussed here? If so, then try re-enabling it. Disabling DMA significantly slows down the speed at which software rendered graphics (you're using the software renderer, right?) gets copied to VRAM.
